
Individual Education Plans are an effective student-centred approach to your child’s education in Middle School. An Individual Education Plan, or IEP, is a written educational plan that documents any changes or adjustments required for teaching and learning for an individual student. These plans are written specifically for your child to address their academic and personal needs. These documents should be realistic, uncomplicated and an ever-evolving document. IEPs are developed with the consultation of a range of stakeholders, which may include teachers, parents, education assistants or other school staff, therapists, and the student.
All staff working with your child will use the plan, so your input in the planning process is very important. These plans will be reviewed each semester to reflect any changes required and to ensure goals are current and relevant.

As part of our senior school transition process, we facilitate person-centred planning with our students using the ‘My Plan’ approach. This will provide a good way for students to develop a sense of direction in their lives and to make plans now and into the future. Family, friends, therapists, support workers, NDIS Support Coordinators, and other people central to your child’s life are invited to meet with teachers and staff to MAP out your child’s future with them and to make plans to achieve their goals.
Students are central to this planning process and will be supported by family, staff and therapists invited by the family to provide as much input as possible. From this, an ACTION PLAN will be developed with roles and responsibilities for identified stakeholders. The plan will be reviewed in Term 3 (Semester 2) and every year thereafter as part of your child’s senior school education.
We encourage key therapists and others who play a significant role in your child’s life to attend where possible.
Teachers will issue a booking schedule closer to the meeting.
Local Area Coordinators can help you to understand and access the NDIS, develop your NDIS plan by collecting information to relay to NDIA, implement your plan and connect you with services, support you in preparing a plan review and link you to community and mainstream services. It is important to know that LACs cannot approve an NDIS plan; this is the responsibility of an NDIA Planner/Delegate.
