Leeming Senior High School Education Support Centre
Leeming SHS ESC welcomes therapists into our school and values the contribution they make to the success of our students and their educational outcomes.
Requesting access during school hours
Request for Access, Onsite Induction, and WHS Induction must all be completed upon request in consecutive order (1,2,3) by the therapist.
Note: Parents do not fill out these requests.
1. Request for Access
Please complete the following form to submit your request to facilitate therapy during school hours in 2026.
2. Onsite Induction Booking
Please complete the following form to book an on-site induction.
3. WHS Induction
Please complete the following form. Once the WHS induction is complete, you will receive an email from Passtab to upload the following compliance documents:
1. Working with Children's Card.
2. Public Liability for no less than $20 million for any one occurrence and unlimited in the aggregate.
3. Nationally Coordinated Criminal History Check or NDIS Workers Screening Check.
